• WEST CENTRAL MN MEMBERSHIP AGREEMENT

    Required Agreement for Participating as a Member of the WC CoC
  • Whereas, the West Central Continuum of Care (WC CoC) requires active, diverse, and region-wide participation to effectively prepare and implement a Region-wide Continuum of Care Plan;

    Whereas, the West Central Continuum of Care (WC CoC) desires participation from members who are committed to preventing and ending homelessness and will commit the time required for professional service; and

    Whereas, , is willing and interested in maintaining membership or becoming a Member of the West Central Continuum of Care (WC CoC).

  • Whereas,   *   is willing and interested in maintaining membership or becoming a Member of the West Central Continuum of Care (WC CoC).

  • Therefore, the above named Member Agency/Organization/Tribe agrees to become a Member of the West Central Continuum of Care (WC CoC), understanding and agreeing to the following membership policies and the organizational code of conduct.

    I.    Membership Policies:
    A. Each Member Agency/Organization may appoint up to three (3) individuals to serve as Member Representatives on the WC CoC;
    B. Additional agency staff, administration, or board members are invited to attend WC CoC meetings and participate on committees;
    C. Member Agencies are expected to strive for 100% attendance at all scheduled meetings/activities;
    D. At each meeting, voting rights shall be given to only one (1) representative from each member agency, regardless of how many staff are regularly in attendance or how many programs the organization administers;
    E. Representatives should come to each meeting prepared to actively participate;
    F. All members are expected to abide by the WC CoC Code of Conduct while participating in organizational meetings or representing the WC CoC;
    G. Member Agencies are responsible for promptly notifying the WC CoC Coordinator of any changes in contact information for their agency, their Member Representatives, their agency HMIS lead, or their CES staff.

    II. Code of Conduct for the WC CoC
    Commitment and professionalism are essential to creating and maintaining an effective and efficient Continuum of Care (CoC) process that will benefit each member of the WC CoC and the homeless individuals and families they represent. Both are integral to creating and sustaining a strong continuum in which collaboration and services can grow.  Conduct obligations include:

    A. Members should strive to represent in a fair, honest and respectful manner their individual agencies and the homeless individuals and families that they are here to represent.
    B. Members should be informed on the purpose of the WC CoC and its role in the CoC process.
    C. Members should strive to stay up-to-date on WC CoC strategies and planning.
    D. Members should strive to fulfill commitments made and to avoid commitments that they may be unable to fulfill.
    E. Members are expected to uphold professional standards of conduct, exhibiting respectfulness, fairness, and honesty.
    F. Members should disclose all organizational connections and any potential conflicts of interest, exercise reasonable judgment, and take precautions to ensure that any potential biases or conflicts of interest do not unjustly affect the CoC process or other members of the WC CoC.
    G. No CoC board member may participate in or influence discussions or resulting decisions concerning the award of a grant or other financial benefits to the organization that the member represents.
    H. When conflicts occur among members, they should attempt to resolve these conflicts responsibly.
    I. Committee chairs and officers have the extra responsibility of setting an example by their personal performance and attitude that convey honest, respectful, and ethical values.
    J. Members should consult with, refer to, and/or cooperate with other professionals and institutions to the extent needed to serve the best interests of those they represent.

  • Please sign this form if you are the authorized representative for your organization participating in the WC CoC. It is not requried that this be the CEO or Executive Director at your agency. However, it should be the person responsible for ensuring your agency will particpate in the WC CoC as outlined in the agreement.

  • CoC Member Representatives
    Each agency may name up to three member representatives to represent their agency or organization. These voting members will receive notification of WC CoC meetings and one representative in attendance will have voting privileges at WC CoC General Membership meetings. The member representatives may or may not include the Authorized Representative of the agency or organization.
